This post lists out each SECP form along with its form code for businesses to easily identify which form needs to be provided when simply asked “Provide Form 29”. How would anyone know which form is Form 29? Keep this page handy if you will be dealing with SECP matters regularly such as obtaining Digital Certified True Copies or Opening Business Bank Accounts.
SECP Form codes
Here is a list of all the forms used by the Securities and Exchange Commission of Pakistan (SECP):
- Form 1: Declaration of compliance with the Companies Ordinance, 1984
- Form 2: Consent to act as director/officer of a company
- Form 3: Application for incorporation of a company
- Form 4: Notice of situation of registered office and any change therein
- Form 5: Consent to act as auditor of a company
- Form 6: Return of allotment of shares
- Form 7: Notice of appointment of auditors
- Form 8: Statement of affairs of a company in voluntary liquidation
- Form 9: Consent of surety to act as such
- Form 10: Return of alteration in the charter, statute, or memorandum and articles of a company
- Form 11: Notice of increase of share capital
- Form 12: Notice of consolidation, division, or sub-division of shares
- Form 13: Notice of conversion of shares into stock
- Form 14: Notice of forfeiture of shares
- Form 15: Notice of alteration of share capital
- Form 16: Notice of reduction of share capital
- Form 17: Return of particulars of mortgage or charge
- Form 18: Notice of appointment or cessation of receiver or manager
- Form 19: Register of members and index thereof
- Form 20: Annual return of a company having a share capital
- Form 21: Annual return of a company not having a share capital
- Form 22: Proxy form for use at a meeting of a company
- Form 23: Special resolution of a company
- Form 24: Annual accounts of a company
- Form 25: Statement of compliance with the Code of Corporate Governance
- Form 26: Power of attorney for use in connection with the winding up of a company
- Form 27: Statement of affairs of a company in liquidation by the court
- Form 28: Order of court appointing liquidator or provisional liquidator

- Form 30: Statement of affairs in the event of winding up by the court
- Form 31: Notice of appointment of liquidator or provisional liquidator
- Form 32: Statement of estimated income and expenditure for the period of administration of a company.
- Form 33: Notice of removal of liquidator or provisional liquidator

- Form 1A – Application for registration of a company under section 43
- Form 1B – Application for the conversion of a partnership or sole proprietorship into a private company
- Form 1C – Application for the conversion of a public company into a private company
- Form 2A – Application for registration of prospectus
- Form 2B – Statement in lieu of prospectus
- Form 3A – Particulars of person(s) or director(s) who ceased to be a director
- Form 4 – Statement of alteration in the company’s share capital
- Form 5 – Notice of increase in the company’s share capital
- Form 7 – Notice of appointment of receiver or manager
- Form 8 – Notice of vacation of office by receiver or manager
- Form 10 – Return of allotment of debentures
- Form 11 – Notice of appointment of debenture trustees
